By signing this contract, I authorize the photographer to freely use the photographic and digital images made of my person.

I declare that I have been fully compensated by receipt of the compensation stated below for my work as a model and will not make any further claims against the photographer or third parties;
As far as the contrary is not stated in this statement, the model guarantees that he/she is of age of majority at the time of the photo shoot, as well as at the time of signing this contract. In the event of a minor, the above parent/guardian declares that he/she is authorized and in agreement.
My data may be passed on to other photographers (third parties) YES / NO
One signed copy of this agreement belongs to the model, a second signed copy remains with the photographer.
